We have been mandated by public health authorities to screen all patients for respiratory symptoms and travel history prior to treatment.

QUESTION 1:

Are you experiencing flu-like symptoms, including: fever, cough, difficulty breathing?

IF YES:
  • Please wear a mask immediately and rub hands with a sanitizer provided by our team
  • Please inform the receptionist
  • If there is no dental emergency, your appointment will be rescheduled
  • Please contact your doctor or Info-Santé (811)
  • We may need to report your case to the infectious diseases duty officer at the local public health department (514 528-2400)
  • For emergency treatment, we may refer you to the CHUM, MGH or Jewish General Hospital.
IF NO:

Proceed to Question 2


QUESTION 2:

Have you traveled outside Canada within the past 14 days?

IF YES:

Your treatment will be deferred until 14 days have passed since arriving in Canada, provided that you remain asymptomatic.

IF NO:

Proceed to question 3


QUESTION 3:

Have you knowingly been in close contact (within 2 metres) with anyone who has travelled to the locations listed above in the past 14 days, or with someone confirmed to have contracted COVID-19?

IF YES: 

Your treatment will be deferred until 14 days have passed since last being in contact with that person, provided that you remain asymptomatic.

IF NO:

We can proceed with your appointment!
